Will I Be Blacklisted for Ending My Contract Early?

I know this might sound kinda selfish, but sometimes you gotta do what’s best for you. Honestly, my take is that jobs don’t really care about you, so why should you care about them? I’m thinking about taking a travel contract in Hawaii or California, but I only want to commit for about 6-7 weeks since I need to get surgery early next year.
I’ve been looking for an 8-week contract, but everywhere I’ve checked wants 13 weeks, probably because of the holidays coming up. I’m considering sticking around until the first week of January to be decent, but I might have to bail early if I sign a 13-week contract.
So, I’ve got a few questions: Will I get blacklisted or marked DNR for leaving after just 6 weeks? If I give a two-week notice after 4 weeks, will that look bad? And what’s a good excuse for needing to leave early?
